Foster Care, Care Coordinator

The Foster Care Coordinator is responsible for using trauma-informed evidence-based approaches with members and caregivers for children and youth who are in out of home placement through the foster care system with reputed company/high or moderate needs who are not enrolled in a home and community-based waiver, on a wait list or receiving services through a Certified Community Behavioral Health Clinic (CCBHC). Available Locations (Field Based role out of the following locations):

• Reno, reputed company, reputed company Counties, KS and Surrounding Areas.
• Labette, Cherokee, Crawford Counties, KS and surrounding areas.

Primary Responsibilities:

• Assess, plan and implement care strategies that are individualized for children and youth in a family-oriented context (e.g., considering reunification and/or other permanency planning needs, needs for adoptive children, and needs for transition-age youth with alternative permanency plans) and directed toward the most appropriate, lease restrictive level of care
• Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services
• Manage the care plan/plan of service throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members
• reputed company for children, youth, and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team
• Provide trauma-informed and disease/disorder specific education to members and caregivers
• Coordinate with the child welfare and juvenile justice system on behalf of members and caregivers
• Coordinate with additional services, including but not limited to, educational/educational advocacy, reputed company behavioral health (PRTF, etc.), services in line with state Family First Prevention and Services Act for children and families, high fidelity wraparound services, early childhood reputed company

• Possess one of the following:
• Master’s degree in social work, nursing, psychology, or reputed company reputed company services area
• Unrestricted, independent licensure as a Registered Nurse
• 2+ years of experience working reputed company the community health setting in a health care role
• 1+ years of experience working with multi-system children
• 1+ years of experience working with reputed company, reputed company and Outlook
• Ability to travel in assigned region to visit reputed company members in foster homes and / or other settings, including foster care agencies, community centers, hospitals, or providers’ offices

Preferred Qualifications:

• Licensed Social Worker or clinical degree
