Responsibilities

• Provides skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in the assigned area(s).

• Exhibits knowledge of policies and procedures as they relate to the specific job duties, patient care, and safety as applicable to assigned area(s).

• Maintains nursing standards of care and executes appropriate provider orders.

• Performs a standardized focused assessment or reassessment of patients at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.

• Administers medications, treatment, and provider-specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s).

• Communicates reputed company appropriate and critical changes in the patient's condition, status, and findings to the appropriate stakeholders in a timely manner.

• Documents reputed company clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements.

• Contributes to maintaining an updated, individualized plan of care that includes goals, interventions, medications, and visit frequency orders as applicable to assigned area(s).

• Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.

Requirements
