ASSESSOR -MY CARE -PART-TIME Status: Part-time (22.5 hours/week) Pay Range: $26,000 - $29,700.00 Work Location: Community/remote based. Must be travel with the COA 5 county area to complete in home assessments. In addition, this position requires being in the office for initial training, required in-person meetings or any subsequent training sessions. Job Summary: The Assessor is responsible for conducting person-centered assessments to determine eligibility for various programs and services. This role involves providing education and support to individuals and their caregivers and ensuring that all documentation and compliance standards are met. The Assessor will work collaboratively with internal staff and external partners to advocate for clients and connect them with appropriate resources. Essential Functions:

  • Conduct person-centered assessments to determine eligibility for programs and services.
  • Utilize Service Guidelines when developing service plans with clients/members and caregivers to promote understanding of service rationale.
  • When applicable, develop initial care plan with individuals and caregivers, addressing their needs and preferences.
  • When applicable, Assist with discharge planning and transitions between care settings.
  • Provide education and support to individuals, caregivers, and legal representatives about available resources and program eligibility.
  • Advocate for clients, particularly in situations involving health and safety risks.
  • Report abuse or safety concerns in accordance with policies and procedures.
  • Work collaboratively with internal COA staff and external partners to ensure appropriate care for clients.
  • Adhere to COA confidentiality policy, Firewall Compliance, and HIPAA protocols as related to all client/member information/interactions.
  • Adhere to all COA and Program policies and procedures. This includes, but is not limited to documentation, program eligibility, and referrals.
  • Utilize electronic documentation systems and other software applications.
  • Maintain accurate and timely documentation in electronic systems.
  • Utilize Service Guidelines when developing service plans/care plans with clients/members and caregivers to promote understanding of service rationale.
  • Create and maintain client/member files.
  • Participate in team meetings, case conferences, and committees as needed.

Required Education, Licensing & Certifications:

  • Ohio licensed Social Worker, Licensed Practical Nurse (LPN), or Registered Nurse in good standing with the credentialing board.

Experience:

  • At least one year of experience in medical social work, home health care, and/or geriatrics.
  • Prior experience completing electronic documentation a plus.
  • A good working knowledge and understanding of issues related to the elderly population.

Skills & Abilities:

  • Critical thinking skills and the ability to effectively use clinical judgment.
  • Ability to learn in-person, through a written format, and virtually.
  • Tolerant, empathetic, good interviewing capability.
  • Knowledge of community resources within Southwestern Ohio.
  • Good customer service skills.
  • Ability to adapt to frequent change.
  • Good oral and written communication skills.
  • Good organizational and teamwork skills, including the ability to collaborate and solve problems in a self-directed team environment.
  • Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
  • Respect the cultural, spiritual, racial, ethnic beliefs of others.
  • High speed internet in own home used for business purposes of accessing agency systems.
  • A valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton and Warren Counties.
  • Ability to travel within the state of Ohio for training.
