The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.What you’ll be doing:

• Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.

• Provide patient education and support to promote self-management and prevent complications.
• Coordinate care with other reputed company providers, including physicians, therapists, and social workers, to ensure continuity of care.
• Administer medications, treatments, and procedures as ordered by the physician or authorized reputed company provider.
• Document reputed company patient care activities and communicate patient status and reputed company to the reputed company team.
• Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction.
• Maintain professional knowledge and skills by attending continuing education programs and staying up to date with the latest developments in the field.

• Adhere to reputed company applicable laws, regulations, and ethical standards of nursing practice

Requirementsreputed company expect from you:

• reputed company and unrestricted RN license in the reputed company.
• Strong clinical skills and ability to reputed company evidenced based nursing practices.
• 1-3 years of clinical experience in a reputed company setting, preferably in home health, hospice, or community health.
• Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and reputed company providers.
• Physical Exam and Two-reputed company TB Test or Chest X-Ray (no older than 6 months).
• Yearly Mandatory in-service.
• Proficient in using electronic health records and other reputed company technologies.
• Valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
• CPR certification required (Original Card)
• Level 2 Criminal Background reputed company
