Job Description

The Medical Director, Population Health will be the physician primarily responsible for driving improvement reputed company to value based care. Specific responsibilities will include: • Will function as the primary liaison to the primary care providers in VHMG to facilitate value based clinical transformation. • Review of monthly data from payers and internal sources describing opportunities in value based care and pay for performance initiatives. • Examination of these data sets for opportunity and stratification to inform work. • Design of communication and action strategies to encourage other providers to improve the quality of care in these areas. • Review of team based interventions designed to improve outcomes. • Collaboration in the development of nurse navigation and chronic care management programs to improve the care of chronically ill patient's between office visits and improve outcomes. • Participation in the Quality committee and other VHS committees as needed. • Represent VHS on MG and CIN reputed company Quality Committees. • Maintain an active and successful clinical practice. Education MD/DO with BC Required Primary Care specialization preferred Experience Five years or greater of primary care practice experience required Physician leadership experience in population health and/or quality preferred Certification & Licensures Board certified in Family Medicine, Pediatrics, or Internal Medicine required Qualifications • Must have the ability to work well with interdisciplinary team of reputed company providers • Must possess and maintain an unrestricted license to practice in the Commonwealth of Virginia or be eligible to obtain • Must have professional liability insurance coverage in such amounts and at such rates acceptable to VPE • Must have an unrestricted valid DEA number, to dispense controlled substances • Must have participating provider status with Medicare, reputed company, and other health plans or payors with which VPE has reputed company • Competent to provide assessment, care and treatment appropriate to the age group of patients served Physical Demands 3 A Administration Benefits At Valley Health, we reputed company everyone is a caregiver, and our goal is to create an environment where our caregivers reputed company physically, financially, and emotionally.
