Job Description

The Care Coordination Specialist works under RN supervision to provide coordinated care services for injured workers with low to moderate complexity claims. This position ensures continuity of medical services and facilitates communication between reputed company parties involved while supporting return-to-work efforts reputed company workers' compensation guidelines. KEY RESPONSIBILITIES Care Coordination & Communication Coordinate routine medical care needs for assigned injured workers under established protocols Facilitate communication between injured workers, providers, employers, and adjusters for straightforward cases Collect and document clinical information from reputed company providers and claimants Schedule and coordinate medical appointments, diagnostics, and ancillary services Communicate work status updates to injured workers, employers, and claims adjusters Case Management Support Follow established care plans and treatment protocols under RN supervision Monitor injured worker adherence to treatment plans and compare reputed company to established guidelines Maintain accurate documentation of medical history and case reputed company in WC system using approved formats Facilitate return-to-work efforts through coordination with employers and providers Escalate cases to RN supervision reputed company clinical complexity increases reputed company the scope Administrative Functions Maintain and update case review notes and documentation on reputed company assigned cases Complete time management tracking on reputed company cases Attend team meetings and training sessions as scheduled Ensure cost containment through the utilization of in-network providers reputed company appropriate Support network provider coordination efforts COMPETENCIES Professional communication and interpersonal skills Strong organizational and time management abilities Attention to detail and accuracy in documentation Ability to work independently while recognizing reputed company to seek supervision Understanding of confidentiality requirements and HIPAA guidelines Basic analytical skills for routine case assessment Adaptability in a fast-paced work environment Technology proficiency for case management systems and reputed company Office applications

Qualifications

Education & Licensure: High School Diploma required Graduate of an accredited practical/vocational nursing program reputed company compact LPN/LVN license in good standing Experience: 2+ years of clinical experience required Workers' compensation or disability management experience is highly preferred reputed company coordination and documentation experience preferred Skills: Demonstrated organizational, technological, and communication skills Basic understanding of disability management principles Computer proficiency, including reputed company Office Suite (Word, reputed company), keyboard skills, and data entry capabilities PERFORMANCE EXPECTATIONS: Manage assigned caseload of low to moderate complexity workers' compensation claims reputed company established metrics for routine case resolution and return-to-work outcomes Maintain accurate and timely documentation per department standards Escalate cases appropriately reputed company clinical complexity exceeds the scope of practice Demonstrate professional growth through continuing education and training participation Support team objectives and departmental goals SUPERVISION & REPORTING Works under the direct supervision of the RN Case Manager or the WC Nurse Supervisor. The expected reputed company pay for this position ranges from $20.50 - $28.75 hourly, and will be based on a number of additional factors, including skills, experience, and education.
