Position Summary The role of the Care Management Associate (CMA) is to support Health Coaches and Nurses, as well as members participating in the Healthy Aging Support Program (HASP). The CMA manages group coaching attendance, schedules appointments, places reminder calls for upcoming appointments and assists members with technical needs related to the group coaching platform. Job role responsibilities also include data entry for members enrolled in HASP. Additional Core Responsibilities Effectively supports Health Coaches during each group coaching session, responding promptly when assistance is needed Monitors and responds to Healthy Aging Support Program voicemails Accurately and consistently documents in member record per workflow Protects the confidentiality of member information and adheres to company privacy policies Ability to be flexible, managing multiple priorities and adapting to change with enthusiasm Determined to build strong relationships with peers

Required Qualifications

Candidate must be willing to support 10 AM – 7 PM EST hours Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted ​Access to a private, dedicated space to conduct work effectively to meet the requirements of the position 1 + year(s) of customer service experience, preferably in the health care industry Experience with computers including knowledge of Microsoft Outlook and Excel Strong organizational skills, including effective verbal, telephonic, and written communication

Preferred Qualifications

Experience with data entry and documentation within member records Bilingual (Spanish) Education High School Diploma or GED (REQUIRED) Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $18.50 - $35.29 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
