Job Summary and Responsibilities Acentra Health is looking for a Service Support Facilitator to join our growing team. Job Summary: The purpose of this position is to facilitate the delivery of service support and administrative functions necessary for effective operation and delivery of services. The Service Support Facilitator is vital to the program's success by conducting initial and annual assessments which are used in determining medical eligibility. **This position is remote, but candidates must reside within the State of West Virginia. Responsibilities: Coordinate and support the delivery of services in accordance with organizational policies and procedures. Assist in managing service delivery processes. Monitor performance indicators and make adjustments as needed. Schedule, conduct and appropriately document member assessments within prescribed timelines. Enter assessment data into the web system. Participate in inter-rater activities to ensure consistency in the administration of the assessment instruments and adherence to assessment protocols. Support in overseeing department procedures and participating in quality assurance/improvement initiatives. Conduct educational sessions for family members and agencies, covering policies related to assessments and budgetary matters. Complete satisfaction survey with a focus on achieving positive results from respondent feedback. Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications Bachelor’s degree in social work or a related field Active and unrestricted licensure for the state of West Virginia as a LSW or higher (LGSW, LCSW) 2+ years of experience with assessments of children and families

Preferred Qualifications

Experience in a healthcare setting Medicaid/Public Sector service experience Proficient understanding of person-centered planning principles Familiarity with various computer applications and adept at navigating multiple screens Demonstrated ability to uphold confidentiality standards and ensure HIPAA compliance when evaluating pertinent issues Strong organizational skills Exhibits a sense of urgency to meet deadlines effectively Why us?
