Key Responsibilities

As an RN Case Manager, you will play a vital role in ensuring our members receive accessible and appropriate care. Your primary responsibilities will include:

Essential and Preferred Qualifications

To be successful in this role, you will need:

• Active reputed company State RN License required.
• Bachelor's degree in Nursing, Health, or reputed company Services preferred.
• Minimum two (2) years of home care experience required, with a focus on care planning, patient assessments, and home safety evaluations.
• Strong experience in care planning, patient assessments, and home safety evaluations essential.
• Prior supervisory experience a plus.
